Surgical Case 2
This lady presented with crowded teeth and a wide space between her upper and lower front incisors. She couldn’t bite her front teeth together and also had a very “gummy” smile. She was embarrassed about the spectacle she made while talking and eating. The treatment plan included the extraction of four teeth to reduce crowding, cosmetic braces to straighten the teeth and surgery to correct the “gummy smile”.

Orthodontic plan
The teeth are aligned, spaces are closed and the front teeth are decompensated so that they can fit together properly at surgery.
Surgical plan
From left to right. First, a wedge of bone is removed from the upper jaw beneath the level of the nasal floor. Then the upper jaw is rotated up at the back and stabilised with wires, plates and screws. This allowed the chin point of the lower jaw to auto-rotate upwards and forwards eliminating the gap between the front teeth.
